Initial Conversation/Referral
During the initial call, our team will be happy to let you know how we can support you in the right way to make you live safely and comfortably in your home. Questions about the person needing care will be asked such as current health conditions and the preferred care times. This is to give optimum care.
Step 01
Comprehensive Need Assessment
This process enables us to meet with the person to be supported physically. During the visit, the needs will be properly and comprehensively assessed, and risk assessment will be carried out. Based on the assessment, a proposed care plan will be created. The funding and costs will be discussed. We accept private clients, local authority funding options, direct payments or self-directed support.

Matching Carers
At Joklad Healthcare Limited, we try our best to assign carer who is a best fit for you based on the information already gathered. We do our best to introduce your carer to you before the start date. This is to build a good start of a therapeutic relationship.
Step 03
Review of Service Agreement
This is where you review and sign the care agreement, schedules, consent forms, and emergency contact information.
Step 04
Commencement of Care Services
Care delivery begins in line with the agreed care plan and schedule. Care plans are reviewed regularly.
